Considering the effectiveness of many factors on patient history, this study with the goal of exploration of emergency physician’s experiences on barriers of precise patient history taking and physical examination was done. Method: This is a qualitative content analysis research. After purposeful sampling, semi structured interviews were held with 18 physicians in Kurdistan University of Medical Sciences. Each interview was recorded and transcribed verbatim and then analyzed using the content analysis method. Results: Based on the participant’s experiences, primary codes of barriers on history taking and physical examination were obtained . Then after integrating or eliminating the overlapping codes, 5 secondary codes or main barriers including: Educational barriers, Cultural barriers, Economic barriers, no knowledge on legal issues, and defect of supervision and administration systems were found. Conclusion: The findings showed 5 main barriers of patient's history writing.
